need help with financial hardship form to make payment arrangements with student loans?

ok so i got my letter of garnishment. and they actually give me some options. i can make voluntary payment arrangements for 15%. i can also request a hearing or i can call the collection agency to make payment arrangements on what they agree on. right now im filling out this hearing form with all my expenses. i live with my girlfriend and her 2 kids. but my girl only works part time since they cut her work hours. on the form they want her ssn- which i dont feel comfortable giving out. i am hoping to get my payments to at least 10% of my disposable pay. i already am paying one student loan with grc collection agency. im in rehab with them. thats $105 a month. should i list my girlfriend on this form? and say im not comfortable giving out her personal info. or should i just list myself on this form only
